PHQ-9: Depression Scale

The PHQ-9 (Patient Health Questionnaire-9) is the world's most widely used screening for depressive symptoms: how often over the past two weeks you've been bothered by typical signs of low mood and depleted resources. It is not a diagnosis of depression but a quick measure of severity. The last item concerns thoughts of self-harm — it is included because it is part of the picture that matters and should not be skipped.

This is a screening, not a diagnosis of depression. The result reflects the severity of depressive symptoms over the past two weeks, not a conclusion about having or not having a disorder.
